I learned about this from the Beeb; there may be better accounts available elseswhere. Representative quote:

The accompanying photo would've given my 10-year-old self nightmares.

It's not the Monster Manual, it's a TV pilot. Journey with the Robinsons as they explore the wonder and danger of the Earth's oceans. Timmy is always getting lost with his pet fangtooth, while Jane wonders if she will ever meet any boys on the ocean floor.
